There is also the danger that in an open group new clients may encounter people who have relapsed, or who have advanced disease. However, after much thought and discussion with the group itself (which is nothing if not democratic) we decided not to segregate the ‘advanced’ clients from the newly diagnosed. This was for two reasons: first, it is doubly difficult for people struggling with advanced disease to be ‘rejected’ by a group they have come to know and trust. Second, the newly diagnosed are inevitably struggling with the same issues as those relapsing – that is, the existential issues of life and death.
COUNSELLOR’S REFLECTIONS One important aspect of our work is to work with clients in their homes. They have so many hospital appointments that fill their weeks that we, more often than not, come to them. Working with clients in a domiciliary setting is challenging; dogs, unexpected callers, toddlers straying in from another room, telephone calls, all disrupt the therapeutic space. Working in a hospital setting is just as difficult. Hospitals are amazingly noisy places where, as will be seen, there appears to be a hierarchy of roles and responsibilities.
Finally, I have to say something about self-care. So far I have been lucky enough to survive breast cancer without relapse for the last fourteen years. If I were to have a recurrence, I would either retire (which at my age would be entirely appropriate) or take two years out before taking up a counselling role again. This is because I believe that in my case emotional issues played a large part in the onset of my disease, and I would therefore have to work on myself again in therapy before feeling able to resume my work with others.
