Whereas volume loading of the heart due to valvular insufficiency or shunt quickly leads to cardiac enlargement, pressure loading due to aortic stenosis, pulmonary stenosis or systemic hypertension does not result in early cardiac enlargement. Concentric hypertrophy, however, does occur in these latter disorders. Enlargement of the entire heart is not always caused by an enlargement of the ventricular chambers; it may be caused by enlargement of the ventricles plus atria or of the atria alone.

Even up until recently there were physicians who challenged the association of coronary artery occlusion and myocardial infarction and believed that necrosis of the myocardium developed first, and that thrombotic coronary artery occlusion was only a secondary effect. However, coronary angiography during acute myocardial infarction has now shown that an occlusion, usually of thrombotic origin, occurs at the onset of nearly every transmural infarction. Coronary atherosclerosis is, in principle, the same disease process which is found in other areas of the body.
